The short version
TB-500 is a synthetic version of Thymosin Beta-4 (Tβ4), a 43-amino-acid peptide your body already makes. In the lab it acts as a major "actin-sequestering" molecule — it grabs onto the building blocks cells use to move and rebuild themselves. That single mechanism is why researchers study it across so many tissue-repair models, from cornea to heart to skin.
What researchers are looking at
When cells need to move into a wound and rebuild it, they remodel an internal scaffold made of a protein called actin. TB-500 binds the loose building blocks of that scaffold (G-actin monomers), which lets researchers study how cells migrate, organize, and crawl toward damaged tissue. This is the central mechanism behind nearly everything else the peptide is studied for.
On top of that, the literature describes Tβ4 as doing several things at once: it's associated with growing new blood vessels (angiogenesis) into an injured area, with calming inflammation by quieting the NF-κB pathway, and with reducing scar-tissue formation (fibrosis). Different stretches of the molecule appear to handle different jobs — researchers have mapped specific amino-acid regions to its anti-fibrotic, anti-apoptotic, and blood-vessel-forming effects.
Most of this comes from animal and cell-culture models, so the findings are best read as what researchers observe in the lab rather than proven human outcomes. What keeps Tβ4 under scientific attention is the consistency — the same actin-regulating, pro-migration, anti-inflammatory pattern shows up across cardiac, corneal, and dermal repair studies.
Cells get moving
TB-500 binds G-actin monomers, the building blocks cells use to remodel their internal scaffold and migrate into damaged tissue.
New blood vessels form
In the literature, Tβ4 is associated with angiogenesis — growing fresh blood supply into the injured area via VEGF and the PI3K/Akt pathway.
Inflammation & scarring calmed
It's studied for quieting the NF-κB inflammatory pathway and limiting fibrosis, so tissue can rebuild with less scarring.

From the lab toward the clinic
Tβ4 has reached rigorous human testing in one specific form: RGN-259, a 0.1% Thymosin β4 ophthalmic (eye-drop) solution for corneal healing. In a randomized Phase 3 trial, researchers reported a 60% complete corneal-healing rate by day 29 versus 12.5% on placebo — a meaningful step from "promising in models" toward controlled human data.
